As Congress debates a repeal of the Affordable Care Act, we might all benefit from remembering what was going on 10 years ago in the health care financing system. At the end of the Bush presidency, as the number of uninsured was increasing, hospitals and physicians were aggressively pushing insurance companies for large increases to cover the costs of providing care to people who were uninsured.
