Giles Yeo: ‘Let’s consider the type of food we eat, and not fixate on calories’

The scientist and broadcaster discusses the drawbacks of calorie-counting and BMI in measuring obesity, and how our growing understanding of genetics is leading to new treatments

Since the dawn of the 20th century, almost all weight loss guidelines have used calories as a simple measure of how much energy we’re consuming from our food. But according to Giles Yeo, a Cambridge University research scientist who studies the genetics of obesity, there’s one problem: not all calories are created equal. In his new book, Why Calories Don’t Count, Yeo explains that what really matters is not how many calories a particular food contains, but how that food is digested and absorbed by your body.

Can you explain why you feel calorie-counting is a flawed approach to weight loss?
There was an American chemist in the 19th century called Wilbur Olin Atwater who calculated the calorie numbers for different foods, by working out the total energy intake you get from them. But his calculations never took into account the energy it takes our cells to metabolise food in order to use it. This is important. It’s why for example a calorie of protein makes you feel fuller than a calorie of fat, because protein is more complex to metabolise. For every 100 calories of protein you eat, you only ever absorb 70.

Countries with high levels of overweight people, such as the UK and the US, have the highest death rates from Covid-19, a landmark report reveals, prompting calls for governments to urgently tackle obesity, as well as prioritising overweight people for vaccinations.

About 2.2 million of the 2.5 million deaths from Covid were in countries with high levels of overweight people, says the report from the World Obesity Federation. Countries such as the UK, US and Italy, where more than 50% of adults are overweight, have the biggest proportions of deaths linked to coronavirus.

Gary Taubes: ‘Obesity isn’t a calorie problem, it’s a hormone problem’

The author of The Case for Keto argues that conventional approaches to tackling obesity and diabetes aren’t working, and that low-carbohydrate diets could be the way forward

Over the past two decades, the UK’s rates of obesity and type 2 diabetes have spiralled, something that has invariably been blamed on our intake of saturated fat. Conventional nutrition science argues this leads to elevated cholesterol levels and a greater risk of heart disease, but journalist Gary Taubes believes we need to rethink this idea. Over the past 20 years, Taubes has suggested that fat has been unfairly demonised, and instead our excessive carbohydrate and sugar consumption is to blame for many of these societal health problems, a concept that has begun to interest increasing numbers of scientists. In his new book, The Case for Keto, Taubes discusses the potential benefits of the ketogenic diet, a low-carbohydrate, high-fat diet that is being studied as a potential treatment for a range of diseases, from obesity and diabetes, to even cancer and Alzheimer’s.

You’ve long been one of the biggest advocates for the benefits of low-carbohydrate, high-fat diets. How did this all begin?
I did an investigative piece for the journal Science back in 2001 on dietary fat and heart disease. I interviewed around 140 researchers and administrators, and I concluded that there was never really compelling evidence for this low-fat diet we’d all been told to eat since the mid-1980s. When writing the story, I had a National Institute of Health administrator say to me: “When we told everyone to go on low-fat diets, we thought if nothing else they’d lose weight, because fat is the densest calorie in the diet. And instead they started eating more carbohydrates and everyone got fatter.” So I always had it my head that one of the main things that caused the obesity epidemic was this switch to a low-fat, high-carbohydrate diet.

In the midst of Covid-19 it is easy to forget that the country is facing not just one, but two, very badly managed public health emergencies. The substantial and largely avoidable death toll in the current epidemic is undoubtedly due to a series of ill-informed and inept decisions about how the country should respond to its greatest public health crisis in more than a century. But the virus’s task was undoubtedly made easier by a serious deterioration in the health of the population over the past decade.

Since the beginning of the 20th century life expectancy in England has improved consistently. Until the last decade that is. As a result of government policies over the last 10 years improvement in life expectancy has stalled, and for women in the most deprived areas it has actually fallen. The widening gap between life expectancy in the best-off and worst-off areas is now almost 10 years for men and seven and a half years in women. Similarly, the infant mortality rate for England and Wales reached its lowest point in 2014 and has been consistently higher ever since. Across a whole range of other public health indicators, such as drug-related deaths, sexually transmitted diseases and childhood immunisations, the position has been deteriorating.

A few years ago, Jenni Murray was out walking with her son and dogs when she saw a potential vision of her future. While she was strolling painfully around the park, stopping to rest at benches where she could, a woman not much larger than Murray passed them on a mobility scooter, her own dogs’ leads attached to the handlebars. If Murray – at 24 stone (152kg) – didn’t do something about her weight, her concerned son said, that might be her before long. How did she feel about herself at that point?

“Extremely obese,” she says. “I was not the fit, active person that I wanted to be. I just lumbered everywhere. I’d had breast cancer and a double hip replacement in my 50s, but it was the obesity that was going to kill me.” It was the final push Murray needed, after a lifetime of dieting, and a warning from her doctor that she was on the way to developing type 2 diabetes. “I thought, I’ve got to do something about it, I’m 64 and I’m not going to make it to 70.” She adds, triumph in her voice, “And I did make it to 70!” She reached the milestone birthday in May.

People who are severely obese in middle age are 50% more likely to die early than those of a healthy weight, according to a large study of UK data.

The research shows people with the highest levels of obesity are running a high risk of a range of serious illnesses and premature death, with 12 times the risk of type 2 diabetes, 22 times the risk of sleep apnoea and nearly four times the risk of heart failure compared to those who are of normal weight.
